Use this form to return/claim IT equipment(laptop,printer,dongle,keyboard,mouse etc)

All request require line manager approval and some may require additional approvals. The delivery Time is an indication of average time to complete this type of request, shown in business days, once all approvals have been given

Please note that the actual time to fulfill this request may be shorter or longer than the Delivery Time depending on current workloads.
